If you have suffered something unexpected during a ZipDX call you can email firstname.lastname@example.org or use this form to report the incident. This form collects the information necessary for us to investigate the nature of the incident.
Please provide as much detail as possible when describing the issue. The more specifics you can give us, the better our ability to discover and address the underlying cause.
Upon submission of a new trouble report our tracking system will immediately send an email confirmation along with a ticket number for follow-up.
